All requests for teachers to complete evaluations and student observation documents must be submitted to the office (not the teacher) 72 hours (weekdays) ahead of the date needed.
Requests of this kind must also be accompanied by a self addressed stamped envelope with the name and address of the medical professional or an email address.
Diocesan policy states that the records be sent directly to the professional and not handed to the parent.
Please note that ACS does not fax records, so an alternative to the SASE is to email the medical professional.
